Please fill out the following information to detail exactly what kind of tattoo you want. This form must be completed in order to move forward with booking your appointment. All forms submitted are answered within 2 weeks. Please always check your spam folder if you haven’t seen a response within this time frame.

Please note, tattoos larger than the average hand size (8″+) may take multiple sessions to complete. Large work like sleeves or full back/torsos may require a larger deposit to book.

I am currently local to the Central Oregon area and can travel to Studio Thirty3 for an in-person consultation upon request. *

If “No”, a consultation may still be required to further discuss your tattoo idea. I can provide Zoom or FaceTime calls as a substitute.
